Job Description

We are seeking a dedicated and experienced Registered Nurse (RN) to join our community-based complex care team. The ideal candidate will have a strong background in Intensive Care Unit (ICU) nursing and a passion for providing personalized care to service users with complex medical needs. Key Responsibilities

1. Provide direct nursing care and manage the overall care plan for complex care service users in the community setting. 2. Collaborate with multidisciplinary teams to ensure coordinated care and optimal patient outcomes. 4. Educate patients and families on health management strategies and available resources, promoting independent living whenever possible. 5. Monitor and evaluate the progress of patients, adjusting care plans as necessary to meet changing needs. 6. Maintain accurate, timely, and detailed records of patient care, adhering to ethical and legal frameworks.

Skills & Experience

1. Current and valid Registered Nurse license and certification. 2. Minimum of 3 years of experience in an ICU setting. 3. Demonstrated knowledge and understanding of complex care and chronic condition management. 4. Strong communication and interpersonal skills, with the ability to engage effectively with patients, families, and care teams. 5. Proficiency in clinical assessments and documentation, ensuring compliance with healthcare regulations and standards. 6. Ability to work independently in the community, with strong problem-solving and critical-thinking skills.
